PART 1

Emergency Contact Information + Applicant’s Consent to Release Medical Information

Before spending your valuable time on these application materials, the very first step is scheduling your tour and meeting with our team to ensure the resident is appropriate for our community based on their needs. If you haven’t done so yet, please click here to schedule your tour before proceeding with the following application materials. Thank you for your interest in joining our family at Light of Hearts Villa!

The first step in the process is a very important one as it allows us to gather all of the crucial information to your care. Once completed and signed, this will allow us to contact your physician regarding the general status of your health, the medications you are currently taking, and the ability of Light of Hearts Villa to meet your needs. You may need to schedule a visit with your Physician at this time.

Please note: history and physical documentation is only current for 90 days, keep in mind your time frame for move-in.

Part 2

Financial Disclosure + Resident Profile

To verify that income guidelines are met, we must receive this completed form in a timely manner. Please do not forget to include copies of your most recent bank statements, investment statements and other pertinent financial documents with that disclosure. 

Before clicking on the button below to begin completing the form, make sure you have the following materials nearby:

  • Copy of Photo ID – front and back
  • Copy of Medicare Card – front and back
  • Copy of Insurance Card – front and back
  • Copy of Prescription Card – front and back
  • Copy of Covid Vaccine Card – front and back
  • Living Will – if you have one
  • POA Document – if you have one
  • POA for Health Care – must have
  • Copies of recent bank statements, investment statements, and other pertinent financial documents

Part 3

Personal Meeting & Assessment

After the physician statement and verification forms have been received, we will set up an appointment for an assessment. We will learn more about you, your interests, your care needs and more.

Part 4

Residency Agreement

The next step is signing the Residency Agreement. This is the legal document required before move-in. We will sit down and discuss it thoroughly so it is not included here on the website. It spells out the financial terms, services provided by the facility, and how to terminate the Agreement. In addition, you will receive a copy of our Resident Handbook, Resident Rights and more. At this time, we will work with you to decide on a move-in date. We will need a check for your first month’s rent and security deposit. If you move in the middle of the month, your rent will be prorated.

Part 5

Move in + Enjoy!

The last and final step will be move-in day! Our team will help to develop a personal care plan that will identify the Resident’s needs and the services Light of Hearts Villa will provide to meet those needs. Enjoy and welcome to the family!
